Fees & Insurance Information

Insurance

I am currently in‑network with the following plans:

• Aetna

• Anthem

• Quest

• United Healthcare (Optum)

• Oxford (Optum)

• Oscar (Optum)

All insurance claims are billed through Headway.

Self‑Pay (Cash) Fees

For patients who are self‑pay or whose services are not covered by insurance, the following fees apply:

• Initial psychiatric intake (60 minutes) - $400

• Follow‑up medication management visit (30 minutes) - $250

• Extended follow‑up or psychotherapy session (45 minutes) - $300

• Crisis or same‑day urgent visit (30 minutes, when available) - $300

Administrative & Non‑Visit Services

Professional services provided outside of scheduled appointments are billed at:

• $75 per 15 minutes

These services may include, but are not limited to:

• Completion of forms or letters

• Review of medical records

• Care coordination with other providers

• Prior authorizations

• Other professional services not conducted during scheduled visits

Cancellation Policy

Appointments are reserved specifically for each patient to allow for thoughtful, uninterrupted care. If you need to cancel or reschedule, please provide at least 24 hours’ notice whenever possible. Appointments canceled with less than 24 hours’ notice, as well as missed appointments, may be billed at the full session fee, as this time cannot typically be offered to another patient. Cancellation fees are not covered by insurance.

Payment & Billing

Payment is due at the time of service unless otherwise arranged. Insurance benefits vary by plan, and patients are responsible for any copays, deductibles, or non‑covered services.
